2d. Electricity/MTNL Telephone/Water Bill/Passport in the name of any Parent or Child
2e. Aadhaar Card/UID Card Issued in the name of any of the Parent

3. Child’s Photograph (4 Passport Size)
4. Mother’s Photograph (4 Passport Size)
5. Father’s Photograph (4 Passport Size)
