You will need .Jpg/.Png files of:
1. Child’s Photograph
2. Child’s Birth Certificate
3. Proof Of Residence (Any Two) of the following:
Aadhaar Card – Parent/Child
Voter Id EPIC Card – Father/Mother
Ration Card (including the name of the Child)
Passport – Father/Mother/Child
Bills – MTNL/BSES/DJB – Father/Mother
